This worksheet is a resource for students planning their Mentored Practice. Ideally, you should complete this worksheet at least one semester before you wish to enter this program. You do not need to have identified a prospective mentor or a prospective field-site to utilize this form. Learning Agreement and Covenant To be considered fully registered for Mentored Practice you must complete and turn into the instructor a Learning Agreement and Covenant Form no later than the first meeting of the seminar. Student Reflection on a Site Visit The student fills this form out after a site visit by a faculty member or the Director of Mentored Practice to their place of ministry.
