Procedure for Background Checks for Mentored Practice Mentors and Mentored Practice Students

1. Please go to the Oxford Document Management Company website.

2. Please go immediately to "Applicants - Click here to complete online
Authorization Form" button.

3. Please fill in Client Code as 1313 for Mentored Practice, then Choose
Position Code 1

4. Complete the Authorization form and payment via your credit card.
The fee will be $160, for which you will be reimbursed the entire amount (for mentors) or half the  amount (for students) in a check from Bangor Theological Seminary.

5. In @4-5 weeks from the time you give authorization, BTS will receive your
confidential background check. All background check records for BTS mentors and students will be held in a safe and confidential file at the seminary.
